To participate in the Direct Purchase Program:
Option 1
- Log into the BV360 Reimbursement Solution and submit a benefit investigation request for your patient.
- Once the patient’s eligibility is confirmed, you will receive a Direct Purchase Program form.
- Talk with the patient about purchase options, and then submit the completed form.
- The pharmacy partner will contact the patient directly for payment, then ship the HA therapy to your office.
- Your office staff will contact the patient to schedule the treatment appointment.
Option 2
If coverage has already been determined, contact your local sales representative to receive the Patient Direct Purchase form.
